Drag & Drop SOAP NOTE

I am reaching out to see if anyone has created a SOAP NOTE in H5P using the Drag & Drop content type? I would like to create one that has the Chief Complaint, Subjective, Objective, Assessment, and Plan sections but will allow for 50 phrase options to be dragged and dropped. I would like the options to fade out once they have been correctly placed and a new option populate in it's place in the list of draggable phrases.
